Motor Vehicle Collision?
In the event that you or one of your loved ones are in a motor vehicle collision, it is important to document as much information as possible. While typically law enforcement officers investigating vehicle collisions will take down information and prepare an accident report, from our experience, often accident reports may include incorrect information or omit important information. To make sure you have all the information that you may need in the event of a motor vehicle collision, we have prepared a "Motor Vehicle Collision" form for your use. Please fell free to download this form and keep a copy handy in your vehicle in case of a collision. We designed this 2 page form, which can be printed on the front and back of a single piece of paper, to help you document key information you may later need.

Don't have a Will, Power of Attorney or Advance Directive ("Living Will")?
It is important for adults to have a Last Will & Testament, and there are many bebefits of having a Will rather than dying intestate. It is also important to have an Advance Directive (also known as a "Living Will") that explains your desires to your loved ones and medical providers in the event that you become unable to communicate your choices regarding receiving or withholding of medical care.
