Statistics suggest that many consumers face substantial financial pressures related to Credit Card Debt, Medical Bills, Educational Loans and Loss of Income. Congress has enacted laws to give consumers debt relief and a fresh start from substantial financial pressures. There is protection from Foreclosures, Evictions, Lawsuits and Prevention of Harassment from Creditors regarding the payment of debts.

The bankruptcy intake form has fields that you can fill in. However, Adobe Acrobat Viewer will not allow you to make changes. One way you can save the data that you enter into the form it is print it as a new PDF file using a free PDF writer utility such as CutePDF Writer. You can download the two free files necessary to use the CutePDF Writer from Acro Software by clicking here and here.
Once you have installed these two files, you can use CutePDF Writer like any other print driver (just select CutePDF Writer in you list of printer options) and then simply save the new completed PDF file to your computer. Then email us the completed intake form that you saved as a new PDF file.

List of Items to Bring to Free Consultation

Please bring the following items to your appointment:
  • A completed Bankruptcy Intake Form (see above).
  • Copies of most recent Federal tax returns.
  • Check stubs for the last 6 months. Each individual pay stub is required.
  • Pre bankruptcy credit counseling certificate. Go to www.cccs-express.org or call them at 1-866-445-2227. The cost is $50. This is mandatory if you are going to file bankruptcy. They will ask for the firm code which is 4799.
  • Credit report or list of all creditors owed including creditors that you are current with. I need the name, address and account number of each creditor. This list needs to include any mortgages and any vehicles that you are purchasing or leasing. Please bring all lawsuit or foreclosure paperwork as well.
